Responsibilities

The Skilled Nursing Intake Coordinator is responsible for coordinating and facilitating the referral and admission process for patients being considered for placement in a skilled nursing facility (SNF). This position serves as a key liaison between referral sources, hospital clinical teams, business development liaison, patients and families, insurance providers, and other healthcare partners.  The Intake Coordinator ensures referrals are processed accurately and efficiently, patient clinical and financial information is obtained and reviewed, insurance and authorization requirements are addressed, and appropriate communication occurs throughout the admission process. The position requires strong organizational skills, attention to detail, excellent communication, and the ability to work effectively in a fast-paced healthcare environment.

Qualifications

MINIMUM EDUCATION:

 

High school diploma or equivalent required.  Associate or bachelor's degree in healthcare administration, nursing, social work, business, or a related field preferred.

 

MINIMUM WORK EXPERIENCE:

 

Two or more years of experience in healthcare, hospital admissions, case management, discharge planning, SNF admissions, insurance verification, or a related field preferred.

Experience with skilled nursing, post-acute care, or hospital discharge processes strongly preferred.

 

REQUIRED LICENSES/CERTIFICATIONS:

REQUIRED SKILLS, KNOWLEDGE, AND ABILITIES:

 

Strong knowledge of healthcare referral, admission, and discharge processes. Familiarity with insurance authorization requirements preferred. Excellent verbal and written communication skills.

Strong customer service and relationship-building skills. Ability to communicate effectively with patients, families, physicians, nurses, case managers, facility representatives, and insurance companies. Excellent organizational and time-management skills.

Strong attention to detail and accuracy. Ability to prioritize multiple referrals and deadlines in a fast-paced environment. Ability to work effectively as part of a multidisciplinary team. Proficiency with electronic medical records, referral management systems, Microsoft Office, and other computer applications. Ability to maintain confidentiality and handle sensitive patient information appropriately. Strong problem-solving and critical-thinking skills.

Ability to remain professional and composed when handling challenging situations.
